The authors present the evidence that tauopathy may begin in sporadic AD much earlier than initially thought. They suggest that early accumulation of tau may have remained uncovered since it occurs in the lower brainstain whilst the earliest pathological alterations in AD are classically explored in cortex and hippocampus. This is important to keep in mind, specifically given the strong correlation between amyloid-beta and tau accumulations.
